My son keep forgetting to bring his water bottle, pencils case home, not focusing in class and disturbing classmates does it look like adhd.how is ADHD dignose?

I have same issue too.. everyday complain from his student care teacher and enrichment teacher..cannot focus, disturbing the class... I suspect his is ADHD child... how to get referral to KKH?
